The Healing of Justinian by Saint Cosmas and Saint Damian — History & Facts

In the mid-15th century, the city of Florence was gripped by tales of miraculous healing, and none was more dramatic than the legend of Saints Cosmas and Damian performing the world’s first leg transplant. Fra Angelico’s The Healing of Justinian by Saint Cosmas and Saint Damian captures this extraordinary moment, inspired by the story of the Roman deacon Justinian, whose diseased leg was replaced overnight with that of a recently deceased Ethiopian. The event, blending medical marvel with divine intervention, was a favorite subject among Florentine patrons, especially the powerful Medici family, who claimed Saints Cosmas and Damian as their personal protectors. At the heart of the painting, Fra Angelico freezes the miraculous operation in a quiet, candlelit chamber.

The two saints, dressed in the robes of learned physicians, bend over the sleeping Justinian, one holding a scalpel, the other guiding the new leg into place. The discarded dark-skinned limb lies nearby, a startling detail that signals both the source of the miracle and the painter’s attention to the legend’s specifics. The saints’ haloes glow softly, distinguishing them from the anxious onlookers, while the gentle play of light on flesh and fabric speaks to Fra Angelico’s fascination with the tangible presence of the sacred.

Unlike the dramatic, crowded scenes painted by Lippi or Ghirlandaio, Fra Angelico’s version is intimate, almost hushed, inviting the viewer to witness the intersection of faith and medicine. Commissioned around 1439 for the church of San Marco in Florence, where Fra Angelico was himself a friar, the painting was part of a larger altarpiece cycle celebrating the saints. Over centuries, the panels were dispersed; some remain in Florence, others, like this one, have traveled through collections and museums across Europe. The exact medium—whether tempera on panel or fresco—remains uncertain, as does the original arrangement of the cycle.

Today, The Healing of Justinian by Saint Cosmas and Saint Damian is recognized as a pivotal work in Fra Angelico’s career, blending narrative clarity with the spiritual intensity that made him a favorite of both his Dominican brethren and the Medici patrons.
